• Reports from care recently provided outside NYU Langone Medical Center
  and the NYU Clinical Cancer Center
• Prior chest X-rays, CAT scans, mammography reports and films, PET scans,
  MRI scans, and bone scans that are available
• Pathology slides and reports
• Results of other recent tests, such as EKGs, blood tests, etc
• A list of questions you have about your diagnosis, treatment options, or
  other issues
